Notice to the Public:

North Central Missouri Mental Health Center operates its programs and services without regard to race, color, or national origin, in accordance with Title VI of the Civil Rights Act of 1964.  If you believe you have been discriminated against on the basis of race, color, or national origin by North Central Missouri Mental Health Center, you may file a Title VI complaint by completing, signing, and submitting the agency’s Title VI Complaint Form.

How to file a Title VI/ADA complaint with North Central Missouri Mental Health Center:

You may file a signed, dated complaint no more than 180 days from the date of the alleged discriminatory occurrence. The complaint should include:

As much detailed information (who, when, why, how) about the alleged discrimination as possible.
The form must be signed, dated, and include your contact information (name, address, telephone number).

A copy of the Complaint Form and the Complaint Procedures can be obtained by requesting a copy from the CEO at 660-359-4487.
